Nurse Practitioner

ROMTech is now looking for a Telehealth Nurse Practitioner to join our Chronic Care Management program. This is a part-time contract position (20 hours/week to start), fully remote, supporting patients with chronic conditions using ROMTech’s innovative telehealth and remote monitoring platform. Flexible, remote, potential for more hours as patient volume grows. Candidates must be licensed in CT or FL.
 

Responsibilities

• Conduct virtual patient assessments and monthly check-ins
• Develop and manage individualized chronic care plans
• Review device and patient-reported data; intervene as needed
• Provide education on disease management, medications, and therapy adherence
• Collaborate with ROMTech clinicians and support teams
