It’s the way you want to be treated.Position Summary: Responsible for regulatory compliance, utilization review, chart audits, performance improvement, and educational development of staff.Essential Duties and Responsibilities:Develops performance improvement priorities for agencyImplements and evaluates performance improvement, quality, and utilization activitiesConducts reviews of medical records (plans of care, clinical and progress notes, etc) for quality and compliance standardsFacilitates interdisciplinary case conferences, monitoring the coordination of care and OASIS data.Monitors OASIS data integrity and completes OASIS data submissionEnsures the development, implementation, and update of the individualized plan of careMonitors and ensures the accuracy of publicly reportable dataProduces monthly reports to monitor quality of services deliveredAssists with on-boarding, training, and educational needs assessments of staffAssists with the evaluation of clinical competence and compliance as needed Supports department customer service and financial goalsConducts reviews and maintains clinical policies and proceduresSupports director with operational functions of the department as neededIdentifies significant issues and possible solutions to department leadershipPerforms related electronic medical record tasks and functionsPromotes a clean and safe environment of care, utilizing the SAFE error prevention habitsProvides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of ConductRequired Qualifications: Education and Training:Licensed health care professionalCOS-C CertificationMinimum 5 years home health clinical experienceIllinois Driver license & car insuranceWork Shift Details:Days, Days -Department:HOME HEALTH ADMINISTRATIONBenefits for YouAt Silver Cross Hospital, we care about your health and well-being and that is why we work hard to provide quality and affordable benefit options for you and your eligible family members.
